We are delighted to welcome you to our office. Please read this important info prior to your appointment.

Since April 2023, the WA state Department of Health stopped requiring masking in Dr offices. If you want to wear a mask, please provide your own.

Please be aware of the following:

  1. To speed up the check in process and to familiarize yourself with our office policies – we recommend you review, print and complete the necessary forms found here.
  2. Please remember to bring your ID, insurance card (example: your medicare MBI card or your Blue Cross/Blue Shield Card) and a method of payment to your appointment. Insurance can be tricky. It’s a good idea to check with your insurance and be sure they do not require a “Prior Authorization”. Otherwise your insurance may refuse to pay and you’ll be financially liable.
  3. We’ve employed digital retinal imaging (DRI) as a method to safely perform complete eye exams to our patients. This has proven to be a valuable tool to screen for retinal disease. Our doctors feel this should be part of every complete annual eye exam and does not substitute for dilation. The DRI screening costs $39.
