WA State Department of Health stopped requiring masking in Dr offices. We no longer provide masks – If you want to wear a mask, please provide your own.
Please be aware of the following:
- You can speed up the check in process and to familiarize yourself with our office policies – you can review, print and complete the necessary forms found here.
- It is very important to remember to bring your ID, insurance card (example: your medicare MBI card or your Blue Cross/Blue Shield Card) and a method of payment to your appointment. Before the appointment we check to see if the insurance you gave us is active (the correct group number and policy number were given to us and we can bill for the exam) but YOU are responsible to know what your insurance will and will not cover. Insurance is tricky. It’s a good idea to check with your insurance and be sure they do not require a “Prior Authorization”. Otherwise your insurance may refuse to pay and you’ll be financially liable. We are contracted with VSP and EYEMED for eyeglass and contact lens exams. If you have a different insurance we do not contract with (Example: Versant health or Superior vision), we can still see you, but you will need to pay for your exam out of pocket and submit it to your insurance plan for reimbursement. Minor patients need an adult to accompany them to their exam.
- We’ve employed digital retinal imaging (DRI) as a method to safely perform complete eye exams to our patients. This is a valuable tool to screen for retinal disease. Our doctors feel this should be part a complete annual eye exam but does not substitute for dilation. The DRI screening costs you $39.
- We reserve the right to charge a no-show fee for missed appointments. If you cannot come, please have the courtesy to let us know.
